From kingfish to sole, tilapia to tilefish, your local fish counter has a wide variety of fish to choose from. The American Heart Association (AHA) recommends eating at least two 3-ounce servings of fish, especially oily fish, per week. Fatty fish high in omega-3 fatty acids that have been shown to reduce the risk of heart attack or stroke include anchovies, mackerel, black cod, salmon, bluefin tuna, and striped bass.

When it comes to choosing the best fish for a healthy diet, it’s also important to consider the potential for environmental pollution. According to the AHA and other leading health organizations, some types of fish can contain high levels of mercury and other chemicals harmful to the environment. These contaminants can negate the heart-healthy benefits of fish and increase the risk of cancer, says David O. Carpenter, MD, professor of environmental health sciences at the State University of New York at Albany.
For your heart and overall health, “the advice shouldn’t just be to eat fish, but to eat the right kinds of fish,” says Dr. Carpenter.
To make sure you’re buying the best fish for your health, read up on methylmercury and polychlorinated biphenyls (PCBs).
Methylmercury is a heavy metal that occurs naturally in the environment. It can also be released into the air as a gas from air pollution. From the air, mercury can enter oceans and rivers, where it turns into a harmful form of mercury known as methylmercury. When fish swim in waters contaminated with methylmercury, the metal binds to their protein (the part you eat).

The Environmental Protection Agency (EPA) and the Food and Drug Administration (FDA) recently updated their recommendations on fish and shellfish consumption by women and children because methylmercury is harmful to the developing nervous system of the unborn child and toddler. A study published in

A meta-analysis published in February 2021 in the journal Environmental Research found that mercury exposure may be a risk factor for cardiovascular disease. In this analysis of 14 studies involving more than 34,000 participants, mercury exposure was associated with an increased risk of heart disease and death. If you have high blood pressure or heart disease, or have had a stroke, a study published in January 2017 in the International Journal of Environmental and Public Health Research suggests getting your mercury levels checked. According to the New York State Department of Health, your doctor can perform a simple blood or urine test.
PCBs are environmental pollutants found in oceans and rivers. But unlike methylmercury, which accumulates in fish protein, PCB accumulates in fish oil. According to the EPA, the main source of PCBs in the diet is fish and seafood. However, PCBs can also be found in red meat, chicken, eggs, and dairy products. After eating food containing PCBs, they accumulate in your body. PCBs are not bad for your heart. However, over time, “they increase the risk of all types of cancer,” says Carpenter.
Why Fish Is A Healthy Protein, And How To Get More Of It
As a general rule of thumb when choosing fish, remember that “young fish is better than old fish, and vegetarian fish is better than carnivore fish,” says Carpenter, because both mercury and PCBs accumulate with age, and are much more abundant if fish eat other fish, not plants.
The EPA and FDA have a list of the best fish to eat and frequency limits for exposure to methylmercury. Because methylmercury can harm the developing nervous system of an unborn baby or young child, the list is intended for women of childbearing age, pregnant and breastfeeding women, and young children. The problem is that there are no PCBs on the government’s list of fish. “There hasn’t been enough science to systematically analyze all fish species,” says Carpenter. Salmon, for example, is high in omega-3s and low in mercury. “But in printed circuit boards, it can be very high,” says Carpenter. Tuna, on the other hand, is low in PCBs but high in methylmercury.
Carpenter recommends eating oily fish that are low in mercury and low in PCBs to enjoy the heart-healthy benefits of seafood’s mega-omega fatty acids without increasing your cancer risk.
“For the most part, the AHA list ignores the dangers of chemicals in fish,” he says. Instead, Carpenter relied on the Seafood Watch guidelines published by the Monterey Bay Aquarium.

In general, for heart health, Carpenter advises knowing your fish and not beating yourself up if you don’t get your two weekly servings. Against AHA recommendations and because of the risk of contaminants, Carpenter only eats fish once a week. “Fish should be part of a balanced diet, but two fish meals a week is not a very wise recommendation,” he says.
